The Cosmic Heart Compact was the third transformation item that allowed Usagi to transform into Sailor Moon and the last item to enable her to transform into her original sailor fuku. It was an upgrade to the Crystal Star. In the anime, the brooch was red, while in the manga and Sailor Moon Crystal, it was pink.
The transformation item Usagi used in Pretty Guardian Sailor Moon was very similar in appearance to the Cosmic Heart Compact, but it was known as the Heart Moon Brooch.

- The Sailor Moon S Nakayoshi Anime Album 1 referred to this item as the "Heart Moon Compact" (ハートムーン・コンパクト), while the second album called the item "Cosmic Heart Compact." The toy version released by Bandai used the latter name.
- This item was seen for the last time in anime episode 130 of the SuperS season, manga Act 39 of the Dream arc, and Sailor Moon Eternal before being upgraded to the Crisis Moon Compact by Pegasus.
