Chaos Galaxia was the name given to the being who appeared in episode 200 of the Sailor Moon anime when Sailor Galaxia's bracelets shattered and Chaos took over her body. Though she was never named in the anime itself, an official trading card gave her name as "Chaos Galaxia."
Chaos Galaxia was similar in appearance to Sailor Galaxia, but her sailor fuku was black instead of gold, and she had black wings, grey skin, and red hair. She fought against Princess Serenity, who refused to fight back, and shattered the Fuuin no Ken. Eventually, the Princess' words of kindness and love, and her unwavering hope got through to Galaxia, and with Serenity's aid, she was able to free herself from Chaos' influence and returned to her true appearance.

- Many fans confuse this form with Sailor Chaos, who existed in the manga, musicals, and Crystal.
- A similar unnamed form of Sailor Galaxia also appeared in the musical Eien Densetsu and its revision, and the musical-only character Sailor Buttress also shared a resemblance to this form.
- Chaos Galaxia showed some similarity to Metalia Endymion in Pretty Guardian Sailor Moon, in that they both sealed a powerful evil entity within their bodies and ended up being corrupted by it.
